To be eligible for disability benefits, all the following must be true:

• You remain unable to perform your job for more than 14 calendar days because of a disabling condition.

• You are a permanent full-time employee of the state of Ohio (some permanent part-time employees of the state may be eligible; please refer to check your union contract).

• You have completed one year of continuous state service immediately prior to the date of your injury or illness.
